iconOpen Gym Rules - Parent Sign Off Form

Starting September 2011, we are requiring that all students must print out the "Open Gym Rules", read the document, place their initials by each rule and have their parents sign-off on the document so that all parties understand that we must have FULL cooperation from all participants.  It is our desire to continue to offer our gym during non peak times to the public but not at the expense of creating a danger to any individual, other customers or damage to the gym.  Also we must insist that each participant be respectful to coaches, customers and to the equipment.  We will keep each signed off document on file and we will ask customers to leave if they violate these rules without any further warning.  We ask that parents use this as a teachable moment for their students.
